As your "Assigned" Patient Advocate, she acts as your liaison, lightening your day to day phone calls and time it takes to make the necessary arrangements. SafeGuardTM Basic for CDPAP Families
SafeGuardTM Home Visits
The SafeGuardTM Basic is a 3rd Party Advocacy and Quality Assurance Homecare Membership. The Patient member must be enrolled in Consumer Directed Personal Assistance Program (CDPAP) or in the process of receiving CDPAP services for eligibility. Additional SafeGuardTM Memberships
The Protective Oversight Partners, Ltd., SafeGuardTM Membership includes regular home visits. Frequency and duration is based on the membership type you select supporting a complete LifeCareTMPlan in the best interest of the consumer receiving services. Each is made to suit the individual participant to fit his/her life and abilities with Primary Care Physician and Designated Representative approval.
